Community Participation Survey

Houston Livable Communities Initiative Survey

The Independent Living Research Utilization (ILRU) program at TIRR Memorial Hermann and the City of Houston Mayor’s Office for People with Disabilities (MOPD) is engaging in a cooperative initiative to improve community living, participation and access to healthcare for people with disabilities and older adults in five relatively disadvantaged Houston neighborhoods (Acres Home, Gulfton, Near Northside, Third Ward, and Second Ward).  With support from Amerigroup, the Houston Livable Communities Initiative will assess neighborhood needs and identify environmental, social and attitudinal barriers that confront residents in the identified groups.  Affected residents will be engaged in the process of conceptualizing and developing work plans for making their communities more livable and more supportive of neighbors who are aging and those who have disabilities.

This research project will provide a framework for public and private agencies to work in concert with community leaders and residents to remove barriers, improve services and help people with disabilities and older adults to have more options for healthy, independent living in their homes and communities.  Finally, the project will serve as a model for other cities and towns to use in building more inclusive and healthier, livable communities.

We would like to hear from you about your experiences in these communities. The questionnaire will take approximately 20 minutes to complete.

Your participation in this study is voluntary and you can opt out at any time. Whether you participate or not will not affect your relationship with ILRU at TIRR Memorial Hermann, the MOPD, or Amerigroup.

This research study involves no foreseeable risk or cost to you. Everything you tell us on this survey will be kept anonymous and confidential.  ILRU, the MOPD, and Amerigroup may review your answers but you will not be identified in the research.
