and affirm that the following is true and accurate, to the best of their knowledge, under penalty of perjury:

Today, March 26, 2011, I visited Patricia Kobylak at the Shore Winds nursing home. Patricia was distraught from the moment I entered her room. As soon as she saw me she said, while heavily sobbing, that she wanted to go home to live with Timothy Bowman, but they wouldn't let her. "They" refers to her brothers, the people primarily responsible for her forced confinement, and the nursing home. She stated this repeatedly while throughout the 45 minutes that I spent with her. In addition, Patricia repeatedly stated that "they" didn't like her. Again "they" refers to her family and the nursing home. I asked Patricia how she was being treated and she said she was being abused. I asked Patricia if they were physically harming her and she said they were. By God's Grace I have been able to visit Patricia since January 2011. Although Patricia has stated repeatedly during every visit that she wanted to go home and live with Timothy Bowman and that her family and the nursing home wouldn't let her go home, didn't like her and that she was being abused which includes being forcefully drugged, this is the most distraught I have ever seen her. Patricia is naturally very jovial, but the years of abuse and neglect have taken a toll on her. I have evidence that Patricia has been forcefully drugged from the time she was first forcefully confined at The Strong Behavioral Health Clinic until now. Throughout this entire process Patricia has been treated like an it; a thing with her NYS Patient Bill of Rights being trampled upon and it is truly heartbreaking. She is a God-fearing Christian woman that wouldn't harm anyone or anything and yet she is being treated worse than a criminal. The only thing that has sustained her is God' Grace, her faith in Christ and the love of her husband Timothy Bowman the man she repeatedly says she wants to live with. As stated above I visited with Patricia for 45 minutes from 1:00-1:45pm. I know that if something isn't done Patricia will die in this facility. I am very indignant about this situation and I want action to be taken immediately. Patricia shouldn't be in that facility and if she dies the family, the shore winds nursing home and her guardian Catholic Family Services will be responsible In Christ, Nicholas Gentile
