First Parish Weston – Children's Program Registration
Welcome to the First Parish children's spiritual exploration & religious education program!  If you have a child of any age (infant through high school!) who regularly participates at First Parish (e.g., nursery, Sunday school, youth group, other children's programs), we ask families to share basic information for our records and in case of emergencies. The form has sections for:
  • Parent/Caregiver and Family Information
  • Participating Children
  • Emergency Contact & Legal forms 
  • Volunteer Sign-Up & Comments
Please be assured that this information will be kept in the strictest confidence, and will ONLY be used for First Parish programs and activities. Pertinent information will be shared with teachers and volunteers (e.g., a child's allergies or learning needs), but other personal details or contact information will not be shared publicly.
