Dear Parents/Legal Guardians,

Thank you for taking the time to complete this important survey about your child’s education and college dreams. Your child’s school is participating in our GEAR UP program and we are requesting your support in filling out this important survey. Our GEAR UP program is funded by U.S. Institute of Education Sciences, and this program provides free services for your children (e.g., tutoring, learning resources, summer camps, and other activities to explore their future college and career pathways). To fulfill the requirement of the funding agency, research surveys are provided to you and your child.

In this survey ‘college’ means education after high school, such as: (a) 2-year community college; (b) 4-year public/private college; (c) occupational certificates; and (d) vocational/trade school. Please keep your child in mind as you are responding to all questions. Your name will only be used for matching student, teacher, and parent survey data. All information gathered will remain confidential and will only be used by Appalachian State University researchers to establish a foundational framework for providing your child with activities to help them advance their education. Your child’s school will NOT be provided access to your answers. Your answers on this survey will NOT, in any way, affect your child’s grades.

The survey should take no more than 15 minutes to complete. There are no right or wrong answers, please just answer as accurately as possible.
