Student Code of Conduct & Program Participation Expectations Acknowledgement
I understand that if I am accepted into the IT Flexible Apprenticeship program, I am committing to:
Abiding by the Columbus State Community College Student Code of Conduct
Acting with professionalism at work and in the classroom at all times
Completing an associate degree and/or a certification credential from Columbus State Community College
Enrolling into a cohort-specific or pre-selected section of Columbus State's First-Year Experience Seminar course (COLS 1100)
Attending an IT Flexible Apprenticeship Orientation
Meeting with the IT Flexible Apprenticeship advisor to schedule first semester courses and attending regular advising meetings
Completing an apprenticeship employment position should I be selected by an employer host
Providing academic, employment, and/or department of labor (DOL) grant documentation as requested/needed
Participating in college-sponsored events such as Networking Night as well as interviews scheduled by the college with prospective employers
Maintaining a GPA of at least 2.0 or above
Student Consent to Share Information with Employers
Columbus State Community College complies with The Family Educational Rights and Privacy Act (FERPA) governing student records (including information on grades, billing, tuition and fees assessments, financial records, and other student record information). The College does not disclose student records or information to anyone (including a student’s parents or guardians, spouse, or sponsor) without the student’s written consent. For more information, you can view the Student Rights under FERPA that are published in the Columbus State College Catalog and the U.S. Department of Education's FERPA website.
I understand that by completing this form, I am authorizing Columbus State Community College to disclose my contact information (academic records, resumes, grades, courses, and grade point average, etc.) to third-party prospective employer partners, that would otherwise be confidential, for the purpose of securing job interviews and arranging potential employment opportunities. I understand that when this information is made available to the third-party prospective employer partner, the College has no control over the use of the records after they are released to them. I understand that I am not legally obligated to provide this consent and that I may withdraw my consent at any time. To withdraw consent, I shall send a written request to Columbus State's Office of the Employer Engagement and Experiential Learning at experientiallearning@cscc.edu. This consent expires when I am no longer enrolled in the IT Flexible Apprenticeship program.

Be legally authorized to work in the United States
Comply with employers who may require a GPA higher than 2.0
Report job offers as well as my employment status and wages regularly (i.e. raises, recognition, work-related challenges/conflicts, loss of employment)
Comply with employer hiring requirements (i.e. drug screenings, background checks, etc.
Apprenticeship Employment Positions Acknowledgement
I further understand that the ITFA program is a great opportunity to build my resume, exercise interview skills, and make connections with prospective employers while also understanding that apprenticeship employment positions are offered on a competitive basis through an interview process and are not guaranteed.

I understand that to be considered for an apprenticeship position, I must maintain a GPA of at least 2.0 or above and complete the required introductory courses as outlined by the major or certificate I am pursuing.
