Guardian Scholars Program Enrollment Form
The SJSU Guardian Scholars Program (GSP), part of the Educational Opportunity Program, is committed to helping students exiting the foster care system, wards of the court, under legal guardianship or unaccompanied homeless minors.  Find out more about GSP online at: http://www.sjsu.edu/eop/gsp/ 

In order to be considered for the Guardian Scholars Program, the following three (3) items need to be submitted at your earliest convenience.

1. This online form.

2. Proof of dependency/verification letter/documentation of foster youth, ward of the court, or homeless verification documents (copies are acceptable).

3. Essay to include answering these prompts:
    --  A brief personal description
    -- The obstacles you overcame in pursuit of your education
    --  Why you hope to attend college
    --  Your educational goals while in college
    --  What you hope to do after college graduation
    --  What you hope to get out of the SJSU Guardian Scholars Program
    --  Do your college and/or high school grades reflect your academic ability and potential?

* Include your full name and SJSU ID on each page of your essay, maximum 3 pages, double spaced, size 11 font.
* Please submit your verification documents and essay together.
* Please email your essay and documentation together directly to guardianscholars@sjsu.edu
