In the interest of safety, we will provide bus transportation for Both Field Trips (Yale British Art Museum, Art Studio Plus in New Haven) *

I will send a check payable to "St. Andrew's Church" to St. Andrews Church; ATTN Arts Camp 2019; 232 Durham Rd; Madison, CT 06443. $185 for rising Kindergarten-8th Grade; $40 for rising 9th-12th grade. To minimize risk and protect your funds, we won't accept cash.

I will pay by credit card. To do so, please visit www.artscamp2019.eventbrite.com (there is a 6.5% credit card processing fee that goes directly to EventBrite) Please note that Sr. Counselors can only pay by check.

We take pictures of campers having fun all week! We want and need permission to do this. *

I give St. Andrew's Church permission to take pictures of my child(ren) and share those pictures in Arts Camp Daily Digests and occasional promotional materials. I understand that St. Andrew's will not share the last name of my child in public promotional materials but might do so in inter-parish communications (ex: church newsletter).

I do NOT give St. Andrew's permission to take pictures of my child. I understand that this might preclude my child from being able to fully participate in some or all Arts Camp activites. I will email Grant Underwood (grant@standrewsmadison.org) to follow up about this. Please include "Arts Camp Media Opt-Out" in the subject line.
