Topic: Lean Root Cause Analysis - Going Beyond Symptoms



APICS Tampa Bay invites local supply chain, operations management, transportation and logistics professionals to our Professional Development Meeting on October 14, 2014 from 5:30 to 8:00 PM at the DoubleTree Hotel, 4500 W. Cypress Street, Tampa, FL 33607.  This will be a joint meeting with Florida West Coast Section of the American Society of Mechanical Engineers (ASME) and the Tampa Bay Chapter of the Society of Women Engineers (SWE).

This is our second annual College Night Event and will be a joint meeting with the USF student chapters of the National Society of Black Engineers (NSBE), Institute of Industrial Engineers (IIE), Society of Hispanic Professional Engineers (SHPE) and American Association of Mechanical Engineers (ASME). Students from the Saint Petersburg College (SPC) student chapter of APICS and BrewsterTechnical Center Industrial Distribution & Management students are also invited. In honor of College Night we will have a special pre-dinner College Internship Program presentation.

Our speaker is Mr. Chet Kagel, Business Process Consultant with the HPK Group, LLC specializing in process improvement and strategic planning consulting.  He will be speaking on the topic of Lean Root Cause Analysis - Going Beyond Symptoms. 

Does your organization experience problems that have been “solved” multiple times but they continue to reoccur?  Each time we implement a solution, the expectation is that the problem is totally solved.  Many times the solution that is used has been tried before – so why do expect it to work this time?

Root Cause Analysis (RCA) enables us to break this non-productive loop.  Root Cause Analysis is not one specific technique but encompasses a group of problem solving methods.  During this meeting we’ll discuss some common methods such as the Fishbone diagram, Logic diagrams, Control Barrier Analysis, and Failure Mode Effect Analysis (FMEA).  In addition, we’ll introduce techniques such as P-M Analysis and Systems Analysis which can be used with more difficult and fleeting problems.  

We’re looking forward to having an interactive meeting, so bring some examples of reoccurring problems that you have experienced and we’ll discuss possible Root Cause Analysis (RCA) tools to solve them.

Speaker: Mr. Chet Kagel



Chet Kagel has over 25 years of manufacturing management and consulting experience with companies ranging from Fortune 100 to venture startups. He has a comprehensive senior management background with a wide variety of manufacturing companies holding the positions of Vice President, General Manager, and Director of Operations. 

Specializing in strategic planning, constraints management, operations  reengineering and a wide range of process improvement techniques he has assisted companies in achieving high performance status. Over the past several years, Chet has used his manufacturing and consulting experience to help hospitals make dramatic cost savings as well.

An APICS Lean Instructor, Certified Six Sigma Black Belt and Certified Jonah, Chet holds a BS in Chemistry from Northern Illinois University and an MBA from Loyola University of Chicago. 

Agenda:

This dinner meeting includes your choice of soda, iced tea and coffee, dinner rolls and a mixed green salad.  Entree choices are Grouper A La Maniere, Chopped Steak, and Chicken Dijon with Garlic Mashed Potatoes and Vegetable Medley.  Dessert is a choice of Cheese Cake or Belgian Triple Chocolate Torte.
