Welcome to CBC Youth Ministry Registration Form. CBC YOUTH exists to glorify God by making disciples of all people through Gospel truths and Gospel living. Our hope is to equip all youth to love God with all their heart, soul, and mind as well as loving their neighbor, community, and church.

In light of our mission statement, we have designed our ministry to disciple your student & family towards greater knowledge and worship of King Jesus. We believe that those who would call themselves Christian take their calling seriously and with resolve. Our hope in this ministry to is to provide the platforms for discipleship and growth.

In order for your youth to be officially registered with our youth ministry, both student and parent must fill out this form and sign the agreements. If you have more than one student in our youth ministry, you must fill this registration form for EACH student separately.
