CCE Tracker - Report a Cycling Incident
This form exists to record cycling 'incidents' with other road users, in the hope that analysis of the data will add weight to campaigns to improve the situation for cyclists.  Full information about what might happen to the data is available at <THIS LINK>, but the key facts are:

>> We hope that the data gathered will be used to add weight to pro-cycling campaigns, however, it will not be distorted to make this the case and we will actively seek to challenge any other organisation who presents a distorted view of this data.

>> Unless you identify yourself within your text, responses are anonymous.  You are strongly advised to report facts only, and to avoid any comments which might harm your case if you end up disputing the incident legally

There are two sections to this form, a required multiple choice element that covers the basics, and a 'further details' bit that will allow better data analysis, but is entirely optional.

We've suggested the times of 'rush hour', but feel free to bend them to match the traffic conditions
Roughly what happened? *
Pick all that apply.  There's a 'more detailed description' box lower down if you've got the time to record it.   Note, a 'left hook' is where a vehicle overtakes and immediately turns left in front of the cyclist, and a 'close pass' is based on your opinion of the speeds involved and whether the distance left for the overtake was safe.

If relevant, what was the number plate of the other vehicle?
Don't use spaces, and replace uncertain characters with a question mark.  Please don't add any additional text, as it makes spotting repeats more difficult
How confident are you on that plate?
This helps us match up any duplicates.  
If relevant, what was the company/organisation of the other vehicle?
Describe the other vehicle
Make, model, colour, condition, driver, etc.  Even if you're confident on the number plate, the description might help others

There is absolutely no assumption of what is right and wrong below (apart from the lights/dark combination!).  Primarily, we're interested about whether the frequency of certain types of incident is affected by what the cyclist looks like, or the environment around them.  Feel free to leave any or all blank.
