Thank you for applying to be a Junior Counselor (JC) at the 2017 CHSAA State Camp in Fort Collins, CO. This completed form and emailed photo must be submitted by May 1, 2017.

You must also have your current Student Council Advisor (if a senior) or your previous year's AC (if you are a returning JC) complete an online recommendation at www.tinyurl.com/CHSAAJC by Monday, May 1, 2017.
