By completing this form it is understood that, should you proceed with a membership application, you will be making the following declaration:

'I hereby declare that I am supportive of the aims and objectives of the Conservative & Unionist Party, willing to be elected a member of the Club and to abide by the rules and the by-laws thereof upon the faith of this my declaration.  In the event of my failing to abide by the rules of the Club or if my views and actions are, in the opinion of the Committee, inconsistent with this declaration, I agree that my membership shall at once cease and determine.'
