Joan Grace Statement as to Disability

You are about to hear statements from Jane Grace, a nurse applying for a pension in recognition of symptoms she claims relate to her service in the Great War. As members of the tribunal panel, you must decide whether or not to advance her claim to the full tribunal panel. You can either complete this form during the performance or directly after. We will also be taking a vote by show of hands once the performance has finished.

1. In what countries did the claimant serve during the Great War and for what periods? In what capacity?
2. Nature of disabilities in respect of which her claim is made. Did she suffer from any of these disabilities, or one like it, prior to joining the Army?
3. State definitely in respect of each separate disability whether she claims that it was attributable to or aggravated by service in consequence of the Great War.
4. Did she receive treatment during her war service? If so, state details of who, where and how she was treated and for what disabilities.                                                                                        
Any other information or evidence presented by claimant in reference to her claim.
