CHS FBLA Membership Form (2017-18)
Hello Prospective FBLA members!

Becoming a member of Cupertino FBLA requires two easy steps:

1) Fill out this form.

2) Turn in $20 in either cash or check (payable to Cupertino High School) to be registered with the national organization. This payment will allow for you to compete at conferences. Please turn in the check or cash in an envelope labeled with your name, grade, and email. You may turn this in to the green box in Room 913 (ask Mr. St. John if you can't find it) or at the next FBLA meeting - which is every Thursday at lunch in the Theater.

Let any officer know if you have any questions, comments, or concerns.
