Outreach teams are an integral community support, and a part of the larger social services picture. In Edmonton there is a need for outreach services, as the older adult population is growing and becoming more diverse.

The Strathcona Place 55+ Outreach Team (SPOT) is a group of caseworkers who’s goal it is to provide supports for individuals in the Edmonton area who are in need- including, but not limited to, the 55+ community. The Strathcona Place 55+ Centre Outreach Team’s goals include making outreach free, accessible, diverse, compassionate, and effective.

This outreach form is a preliminary intake measure for out caseworkers to assess the needs of the individual. This form does not require confidential information. However, it does requires general contact information. In this manner, we can intake, asses, and contact clients in a timely and private manner.
