Complete one form per family if you are planning on leaving your child at Servant Church during the Bar of Fun Goodbye Party. Children are not allowed at Bar of Fun.

* Childcare drop off begins at 7:00 pm. All Children need to be picked up by 9:30 pm.
* Please feed children dinner before they arrive.
* Simple snacks will be provided.
