INCLUSIONS: 8 workshop sessions, 1 photo shoot, 1 showcase kit, 1 ticket to showcase

TERMS AND CONDITIONS *

Participant is required to be present on scheduled rehearsals, general rehearsal, technical dress rehearsal and on the show itself on 03 June 2023 at UP FILM CENTER, UP DILIMAN QUEZON CITY. Failure to be present on any of said activities for any cause, does not entitle participant to any refund, partial or full.

Payment does not cover cost for costumes, props, hair and make-up. The same shall be subject to the creative discretion of your teachers guided by this year’s theme DANCING THROUGH THE DECADES

Participants shall make themselves available on MAY 14, 2023 for the photoshoot. Exact time slots per groups shall be released together with the guidelines one week before the shoot. Unavailability on set date does not merit reduction of recital fees / refund of any amount paid.

For participants paying by installment terms, a non-refundable down payment of Three Thousand Pesos (P3,000.00) shall be collected upon signing of this form. The remainder of the balance shall be paid on or before the scheduled shoot on May 14, 2023. Failure to pay in full before said date forfeits participant’s slot for the photo and video shoot.

Participants are strictly prohibited from sharing or posting online videos or clips of your production number rehearsals before the day of the show itself.

Participants guarantee their physical health and that they are cleared to participate and perform activities required in their respective production number of choice. Elite Aerial Arts will not be held liable for any accidents or injuries that may be incurred during rehearsals.

Elite Aerial Arts reserves the right to dissolve or combine workshop classes if the minimum number of participants are not met. Due to the current situation, I understand that dates may be moved and adjustments may be made to schedules in relation to DANCING THROUGH THE DECADES keeping in mind everyone’s health and safety . Elite Aerial Arts will keep an open communication on updates should there be any changes.

I understand that Elite Aerial Arts has a COVID policy and protocol that must be followed during rehearsals, photo and videoshoot and other activities. I agree to follow said protocols which may include but not limited to taking tests before participating in activities, wearing of facemasks during rehearsal etc.
