2019-2020 NHS Passenger Request Form
This form is one of two steps for a student driver to get permission for a non-immediate-family passenger.  Siblings do not require any special permission beyond including them when registering as a Driver online.

If there are siblings you wish to have included as passengers, they can be requested on a single form.  However, if you wish to have passengers from different families, you must complete this form separately for each.

Completing this form does NOT grant permission for a passenger.  The passenger's family must also complete a form at: tinyurl.com/NHS-PassengerPermission

A student who brings unauthorized passengers to school risks incurring disciplinary action for them self and their passengers.

Reason for coming along as a passenger?
With lesser-experienced drivers, more passengers in a car increase the risk of an accident.  To focus on safety, school policy requires passengers to have a valid reason for being included.  This might be a shared sport, club, or employment.  Reasons such as "They are my neighbor" or "They are on the way", etc will not be accepted.
