General Waiver Form

The following information will be used to complete a document that you will read and then sign.

IMPORTANT, PLEASE READ THIS FIRST

I understand and acknowledge the nature of the sport that I / my child or ward will be participating involves risks and self-judgement. I hereby certify that I am / my child or ward is physically and mentally fit and I do/ my child or ward does not suffer from any illness, disorders or chronic diseases.

DECLARATION OF INDEMNITY:

I hereby undertake, at all times, to indemnify, defend and hold harmless T-Hall.sg, its management, officers, agents and employees from any and all liability, loss, damage and expenses (including attorney fees) they may suffer as the result of claims, demands, costs or judgments which may be made or instituted against them or any of them by reason of personal injury (including death) to any person or damage to property because of, arising out of, or connected with the course, activities, programmes or my participation therein. It is understood and agreed that such indemnity shall survive the termination of this agreement.

I agree to abide by all rules and regulations establish by T-Hall.sg on the usage of equipment, facilities, services and all other amendments or alterations thereto including all signs, posters, billboards and all written and verbal instructions notified from time to time.

I authorise T-Hall.sg and any other advertising or marketing agency on behalf of T-Hall.sg to take, use and/or reproduce photographs, films and/or digital media of me during the usage of the facilities and/or services for any purpose deemed suitable by T-Hall.sg.

I acknowledge that T-Hall.sg shall only be responsible for the acts of its employees or any other entity controlled by T-Hall.sg on the management, control, supervision, directions and/or operations of the facilities and/or services and shall not be liable for any acts done without instructions/supervision/authorisation from T-Hall.sg.

I declare that all information given in this form is true and correct to the best of my knowledge and belief and note that the data collected above will only be used by T-Hall.sg in accordance with their Personal Data Protection Policies.

I acknowledge that I have read and understood all the matters referred to, in this agreement herein contained.

PARENT’S/GUARDIAN’S CONSENT FOR PARTICIPANT BELOW 18 YEARS OF AGE:

I hereby allow my child or ward listed above to climb in the premise of T-Hall.sg. While I understand that every effort will be taken by the owner to ensure the safety of my child or ward, I undertake to accept that no liability will be borne by T-Hall.sg, its management, crews, staffs and organisers for any personal injury or loss sustained by my child or ward during the activities, with the full understanding that all necessary precautions will be taken to ensure the safety of all participants and therefore release T-Hall, its management, crews, staffs and organisers from all or any liability of any nature which may arise from my *child or ward’s participation in the activities.
