Please include an email (preferred) and/or phone number where we can contact you or we will not be able to follow up with you. Include a "Performer Bio" if you have one. If selected to perform, this will be read by our hosts before you perform on the day of the festival. If you have videos or recordings of performances you have done in the past, please share them in the "Sample of Work" section. This helps our committee get a better understanding of what you do. 

Please note that submission of this form does not necessarily guarantee that you will be selected as a performer. Our committee will review what you submit and reach out with an approval or letting you know that we will not be hosting you on our main stage this year towards the end of April/beginning of May. Keep in mind that the Juneteenth Festival is a family-friendly event - anything that is not that will not be selected.

Once selected, we will reach out to you via email with a time slot for the day and specific information on how to bring music if your performance requires.
