Please fill out the information below to be emailed the full enrollment form for the Montgomery Middle School Before and After School Program Club MAYA! Your student will NOT be enrolled until the FULL ENROLLMENT FORM in CITY SPAN is completed.k  

Before School Hours: 6:30 am-8:10 am, Monday through Friday
After School Hours: 3:10pm-6:10pm, Monday-Thursday and 1:05 pm-6:00 pm on Fridays

*all students interested in playing sports must be registered in Club MAYA. Also, all students must attend at least half the program time each day to stay active.

If you would like to enroll your student in the before and after school program, Club MAYA, please fill out our interest form (step 1). We will then email you the registration packet to complete (step 2)! This is a first-come, first-serve opportunity and open to 150 students. If your student is put on the wait list, we will contact you. If you are NOT contacted, your student is registered and ready to go! Club MAYA will start on the first day of school, July 21, 2021.
