FULL MEMBERSHIP FORM. The form is designed to allow us to better know our members and the state of OT Practice in the Philippines. This will also serve as a research data gathering form in order to come up with a survey of OT practice in the country and to help in policy making and further advancement of the profession. This form will collect information including but not limited to your personal details, educational background, and specific details about your practice. Filling out this form may take approximately 15-30 minutes of your time. This is a one-time fill-out form. Kindly utilize this form as well if you would like to change your stored information.

RENEWAL SECTION. This form is for members who have an expired or expiring membership and wish to renew their membership with PAOT. A filled-out full membership form is required prior to use of this section. If you have filled-out the full form before, there is no need to perform such again and you may proceed with the renewal form.
