Signature (By entering your name), you are agreeing to drop your child off no earlier than 12:45pm and to pick them up no later than 3:15pm. You are also authorizing EFBC to seek medical treatment for the child(ren) you leave in our care in the event of an emergency. Further, you agree to hold EFBC faultless for any accidents or injuries the child may sustain in our care. We love children and will do our absolute best to care for them!  You also agree that if your child is 4 or 5 years old, you will remain on site and monitor your child. You also affirm that your child is between the ages of 4 and 11. *
