Signature (By entering your name), you are agreeing to drop your child off no earlier than 12:45 and to pick them up no later than 3:15pm. You are also authorizing EFBC to seek medical treatment for the child(ren) you leave in our care in the event of an emergency. Further, you agree to hold EFBC faultless for any accidents or injuries the child may sustain in our care. We love children and will do our absolute best to care for them! However, there are risks associated with a 24' water slide and we want you to be aware that by signing you are agreeing to hold EFBC faultless for any accidents. You also agree that if your child is less than 5 years old, you will remain on site and monitor your child. Additionally, you give EFBC permission to use any pictures that are taken, which may include your child, in social media posts or its own website. *
